Brain Injury Rehabilitation

Tordan Healthcare provides an individual tailored rehabilitation service at home for people with Acquired Brain Injuries. We have experience in provided person-centred acquired brain injury care and support with varying severities including traumatic brain injury support (TBI), acquired brain Injury (ABI) hydrocephalus support, and after stroke care.   We work closely with families, specialists, rehabilitation centres, therapists and other health organisations.
A Flexible Approach to Brain Injury Rehabilitation
Care Needs Assessment and Support plan focused on neuro-rehabilitation activities to re learn day to day tasks.  We offer support with a variety of individual needs such as daily living skills, social interaction, confidence building, emotional support and advocacy.

Hospital to Home Service

In most cases Palliative Care or end of life support is required for clients who have been discharged by the hospital or care facility as they have made chosen their preferred of care is to remain at home.  Tordan Healthcare, on receiving a request to support a client discharge, is able to implement a rapid response care needs and support plan to ensure a smooth transition between discharge from hospital and arrival at home, while providing support for the client and their families.

Live in Care

Live In Care

Tordan Healthcare provide live in care services across the city of Manchester and the counties of Cheshire and Derbyshire. Live in care as the name suggests is when our team of highly trained careers live alongside patients in their own home.

We have experience in supporting adults with a range of complex live in care needs and congenital conditions. Care packages are available on a short or long term 24 hour live in care basis dependant on the needs of the person.

Tordan Healthcare manages care for children and young people on behalf of local authorities, CCG’s and private clients.   We have experience and knowledge working with children across a wide range of specialisms for example; autistic spectrum, sensory impairment /multi-sensory impairment, severe and partial physical disabilities, profound and minor learning difficulties, terminal illness, complex care, coping with challenging social environments.

Complex Care

For clients with complex needs the emphasis on care and support coordination and multi-disciplinary working, proactive-ness, independence and contingency planning will be more prominent. 

For clients with less complex needs there will be more emphasis on self-management, independence, knowledge and prevention.

Our clinical infrastructure means that we have a Quality Assurance and Compliance Manager, Case Managers and Care Managers with a wealth of combined knowledge,  professional expertise and community experience across a wide range of clinical conditions.  Our specialisms include tracheostomy care, ventilator care, complex continence needs and gastrostomy care that have seen Tordan Healthcare provide specialist solutions across many neurological conditions as well as acquired brain and spinal injury.
